    American Diabetes Association certification to provide DSMES and bill health insurance.

    DSMES is nutrition and self-care behavior education leading to optimal blood sugars. Wellness 4 Prevention partnered with Your Diabetes Partners, Tammy Mische and Susan Berberich, both RN, CDCES with over 25 years of experience.

    With a provider referral for DSMES, patients may receive up to 10 hours of education the 1st year and 2 hours of education the 2nd year.

    Choose level of support: weekly, bi-weekly, monthly, 6 months or 12 months.

    Complete intake forms.

    Initial session includes review of medical history, height, weight, waist and hip measurements, recent lab work, medical symptoms questionnaire, physical activity readiness questionnaire (PAR-Q), and review 3-5 days of food logs.

    Provide calorie and macronutrient targets.

    Set 1 or 2 goals for the following 1-2 weeks.

    Schedule follow-up sessions for ongoing nutrition, fitness and health education to support personal health goals.

    Medical Nutrition Therapy - MNT

    Accepting patients ages 10-100 who want to improve their nutrition and prevention chronic disease.

    Carolyn has 28 years of experience working with high risk individuals.

    Blood work within past 6 months is recommended. Estimated Glomerular Filtration Rate (eGFR) between 30 and 44 mL/min/1.73m2 qualifies as chronic kidney disease (CKD) 3b.

    Session includes evaluation of current dietary practices, health challenges, lab values, wellness goals and scheduling of weekly, bi-weekly, monthly sessions.

    Provide nutrition support to decrease risk of chronic diseases - heart disease, diabetes, high blood pressure and/or obesity.

    With a provider referral, will bill insurance for Medical Nutrition Therapy. Currently, Medicare covers MNT for CKD 3b and or Diabetes Self-Management Education and Support.
