Reduce the risk of prediabetes, type 2 diabetes, heart disease and other chronic diseases by partnering with local community organizations, faith-based organizations, and medical and dental providers to offer nutrition consultation for all age groups.

Target Audience

Seniors, adults and adolescents who want to be with their family and friends for many years to come.

Persons ready to successfully manage their prediabetes, diabetes, cholesterol, blood pressure and/or change body weight.

Diabetes Prevention Programs starting August 2024!

Fully covered for eligible Medicare and Medicaid individuals.

Self-pay of $910

26 1 hour sessions, gaining the knowledge and implementing lifestyle changes to reduce risk of type 2 diabetes.

Locations: The View Community Center, Grandview, MO and Gamber Community Center, Lee’s Summit, MO.

Virtual participation is also an option, with working internet services.
